REQUEST VISA LETTER

To request a letter from the Academy of Laser Dentistry to support your application for a U.S. Visitors Visa, you must first register for the ALD 2024 Conference & Exhibition HERE. After you register for the conference, please complete the Visa Letter Request Form. This form is needed to ensure the accuracy and spelling of your submitted information. A letter will be sent to you with the proper information. This letter is not a substitute for your actual Visa. You must still comply with the formal visa application process.

INSTRUCTIONS

Please follow these instructions:

Prior to completing this application, you must first register for the ALD 2024 Conference & Exhibition HERE.
Complete and submit the requested information on the form. Use your own unique email address so that we can contact you directly, if needed. Paying careful attention to these rules will expedite your visa request.
The submitted form will be sent to the ALD office for our records.
You will immediately receive confirmation on your browser that the form was delivered. An email confirmation will also be sent.
Finally, once we verify that you have registered for ALD 2024 and we have all of the required information, you will receive a formal letter of reply from the Academy of Laser Dentistry office. This may take 2-3 business days.
