COVID-19: Speculation on Possible Effects of Photobiomodulation (PBM)
James Carroll, AMInstP, FRSM

No research has been published to-date with PBM on COVID-19 patients, and none is in progress that we are aware of. However, hypothetically there could be some benefits from PBM treatment of symptoms and conceivably improved innate anti-viral immunity. PBM is well known to be immunomodulatory in cell and animal studies though no work has been published specifically on anti-viral immunity in humans or animals. There is encouraging data that PBM reduces the painful symptoms of a herpes zoster infection and herpes simplex eruption, which while interesting these are not respiratory coronavirus infections. What may be more relevant are the anti-inflammatory effects. PBM is a potent antioxidant. It seems to reduce the underlying cause of inflammatory cytokines (including IL-6 a leading player in the cytokine storm), triggering the Acute Respiratory Distress Syndrome (ARDS), the leading cause of death from COVID-19. Evidence from in vivo models of LPS induced ARDS shows that PBM significantly reduces pulmonary and extrapulmonary inflammation. The research shows there are changes in mRNA levels in the genes involved in the apoptotic process and the DNA fragmentation of inflammatory and alveolar cells.

Educational Objectives

  • Understand how PBM may improve innate immunity
  • Understand how PBM reduces inflammation
  • Understand how to apply PBM to achieve these effects

Using Lasers to Safely Treat the Medically Compromised Patient
Mel A. Burchman, DDS, MALD

Treating the medically compromised patient is my passion and my love. It is the best thing I have done in 45 years of practice. My hope is that many medical/dental professionals will watch this presentation and become stimulated to join with many laser practitioners to the benefit of our mutual patients. We are finally becoming more educated and aware of the link between the oral and systemic health of our patients.

In today’s modern dental practice we come into contact with patients that have very complex and compromised medical histories. Lasers have allowed me to treat many of these patients in my office while keeping them on their daily medications and eliminating the need for additional hospitalizations. All of these cases have been in conjunction with the patient’s medical doctors.

My cases will demonstrate laser techniques that you can incorporate into your practice. You will learn my technique for preventing and decreasing the symptoms of oral mucositis. How to use lasers to treat emergency patients that are currently undergoing chemo/radiation therapy. You will learn my pre and post-op regime for extractions and root canals I will show how to treat patients with many varied medical conditions. Some of the conditions these patients had are: Cancer, Secondary Myleodplastic Anemia, and other Blood Disorders, Pulmonary Embolisms, Heart Surgery, Severe Lymphedema, Cicatricial Pemphigoid, Parkinson’s disease, Stroke, and Neuro-disorders. Optimal Timing of Release: Identifying Comorbidities in Babies Needing Tethered Oral Tissue Release
Michelle Emanuel, OTR/L

Identification of Tethered Oral Tissues (TOTs) is an important step in addressing feeding problems in pre-crawling babies, however, identification is not the indicator for release. Signs and symptoms of neurodevelopmental vulnerabilities present themselves in posture, oral, airway, facial, and gastrointestinal problems. Preparing babies and families for release through astute assessment and treatment is a necessary component for optimal outcomes. This presentation will address these concepts so that attendees will learn ways to incorporate them into their laser dental practice.

Educational Objectives

  • Specify 3 comorbidities with tethered oral tissues.
  • Identify 2 aspects of optimal timing of release of TOTs.

The Speech-Language Pathologist’s Integral Role on the TOTs Team
Vanessa Anderson-Smith, MA CCC-SLP COM

Many professionals see TOTs in the pediatric population as a feeding issue in infants or a speech production/feeding disorder in older children. It also has the potential to contribute to a myriad of health implications. A speech-language pathologist will retrain a child to use muscles that the child may have never had (or had limited) access to prior to a frenectomy. Post-frenectomy therapy will assist with integration of the new motor patterns into safe and efficient eating and clearer speech production. A speech-language pathologist’s role in pre- and post-frenectomy care is essential for neuromuscular reeducation. The benefits are many, including reaching optimal results for improvement in a child’s feeding and speech, as well as enhancing other health benefits.

Educational Objectives

  • Understand the role of a speech-language pathologist in pre- and post-frenectomy care.
  • Describe the need for proper neuromuscular reeducation post frenectomy.
  • Summarize the impacts of TOTs on speech production and feeding.

CBCT and Computer-Guided Implant Placement
Robin Horton, BDS

Clinicians now have access to relatively low-cost CBCT machines, intraoral scanners are commonplace, and labs now have model scanners. If we merge this data and superimpose the proposed final restorations over the three-dimensional computer model, we have the opportunity to use very minimal surgery with very low risk to the patient. These technologies are ideal for laser users, as minimal intervention is required, and tissue sculpting with a laser improves both the aesthetic outcome and healing of the surgical site.

Educational Objectives

  • Understand the importance of CBCT in implant planning.
  • Describe how to use software and scanning technology for designing guided implant surgery.
  • See how lasers fit into a computer-guided surgery protocol for implant placement and restoration delivery.

Combining Lasers, Regenerative PRF, and Ozone Therapy in Endodontics Using EndOzLase: New Biological Regenerative Strategies for the Endo Patient
Valerie Kanter, DMD, MS, BCN, IBDM

Regenerative Endodontics is defined as biologically based procedures designed to replace damaged structures of the pulp-dentin complex. Depending on the state of pulpal inflammation, a variety of treatment options exist that range from direct pulp capping to complete revascularization of the root canal system.   An accurate pre-operative diagnosis is critical for case selection and successful outcomes.  In this webinar we will review clinical guidelines for treatment planning vital pulp therapy cases and learn about new research in the field of molecular diagnostics, to establish a true understanding of the dynamic role of the host response in pulpal and systemic inflammation.  In order for vital pulp therapy to be successful, biological regenerative strategies are recommended to mitigate inflammatory triggers and support mineralization pathways.

EndOzLase is a combination therapy using Er:YAG and Nd:YAG lasers, ozone therapy, and regenerative platelet rich fibrin. We will discuss the application of EndOzLase in vital pulp therapy cases, with the intention of maintaining pulp vitality, which is essential for dental homeostasis and optimal function and survival of teeth.

Essential Legal Strategies for Reducing Taxes, Avoiding Lawsuits & Protecting Your License

Natalyn Lewis, Legally Mine, N. Orem, UT, USA

Dental and medical professionals have unfortunately become all-too-easy targets for MANY trial attorneys! Learning how to use legal entities is vital to protect everything they have worked hard to achieve. Setting up C-Corps, S-Corps, and LLCs is only part of the answer to this rapidly growing issue. This course will instruct attendees on how to protect one of their most important assets, their dental or medical license. Even if they aren’t sued for a larger amount than what their malpractice insurance covers, all dental and medical professionals will encounter being investigated and possibly sanctioned for issues UNRELATED to the reported lawsuit. Utilizing these legal tools, in many cases, can decrease malpractice insurance costs and drastically reduce their income taxes – all while keeping their name and license from being reported to the National Practitioner Data Bank (NPDB).

Educational Objectives

  • Maintain the focus on improved patient care rather than malpractice defense.
  • Improve overall professional success by structuring assets for lawsuit protection and prevention.
  • Apply risk management techniques that improve and enhance fiscal efficiency.
  • Acquire an understanding of basic dental and medical-legal tools that are available to help decrease unnecessary loss of revenue.

Laser Therapy in Orofacial Pain

Daiane Thais Meneguzzo, DDS, MSc, PhD

Orofacial pain is a very common complaint in the dental office, in particular Trigeminal Neuralgia. Pharmacological treatment has been the only option available for a long time. The side effects of prescription drugs are numerous, especially for patients with chronic pain. Temporomandibular dysfunction is another complaint that requires adequate clinical diagnosis and often the use of pharmacological drugs.

The use of Laser Therapy has been shown to be effective in resolving and managing pain by accelerating the resolution of the inflammatory process, modulating inflammatory mediators, and avoiding or decreasing the use of drugs as well as reducing treatment time. This presentation will show the mechanisms of action of phototherapy in orofacial pain, the benefits of lasers as an adjunct in the treatment of orofacial pain, and the clinical management of the use of class 4 lasers.

Educational Objectives:

  • Managing neuralgia with Laser Therapy
  • Managing TMD with Laser Therapy
  • Reducing the use of opioids in chronic pain with Laser Therapy

Managing Inflammation (during a Pandemic!)

Tanya Dunlap, PhD

Treatment for periodontal disease has always been a little like a carousel ride, and now add all of the complications of a pandemic. Laser therapy is an excellent way to treat gingival infections without aerosols, but what do you offer patients between office visits? The biofilm just grows back. In a Covid-19 world, a healthy immune system takes on new significance and any chronic inflammation taxing the immune system needs attention.

Non-aerosol prescription tray therapy delivers hydrogen peroxide gel deep into periodontal pockets to fight gingival infections and to decrease inflammation, bleeding, and pocket depths. It’s a great adjunct to laser therapy and it solves a few problems. You can prescribe trays for patients with delayed care (you just need someone to take an impression) and offer a proven therapy until they can get an appointment. Because hydrogen peroxide kills coronaviruses, this peroxide-based therapy can be implemented to reduce the risk of oral transmission of the virus via saliva.

Come to the course to learn how to integrate Rx tray therapy into your periodontal protocols and how to add an extra layer of protection for your team and your patients.

Educational Objectives:

  • Identify the benefits and limitations of hydrogen peroxide for biofilm control and what we know about peroxide and coronavirus.
  • Summarize how and why antimicrobials with oxidative and oxygenating reactions more effectively manage biofilms.
  • Differentiate the two protocols for Rx tray therapy and good patient candidates for each.

Tanya Dunlap, PhD is the Managing Director at Perio Protect, where she has worked since 2005. For several years she served as the research liaison for the company, working with researchers and investigators who were testing Perio Tray® therapy in controlled clinical trials and microbiological studies. Her expertise on this adjunctive periodontal therapy is grounded in the data, explaining how the prescription trays work and which patient candidates can benefit from them.

Perio Protect LLC  markets the prescription Perio Tray® and provides CE on minimally invasive periodontal treatment with adjunctive Perio Tray therapy.  Results from Perio Tray delivery include reductions in inflammation and bleeding, decreases in pocket depths, whiter teeth and fresher breath. More than 20,000 dental professionals have attended Perio Protect courses and hundreds of thousands of patients have received prescription Perio Trays since the company’s founding in 2005.
